APPOINTMENTS
It is the patient’s responsibility to schedule and confirm their own appointments. Patients must provide at least 24 hours notice for any cancellations or rescheduling of appointments. Failure to do so may result in a cancellation fee.
Insurance
Patients are responsible for providing the clinic with accurate insurance information and for understanding their own insurance coverage. The clinic will assist with insurance claims, but any unpaid balance will be the patient’s responsibility.
Payment
Payment is due at the time of service. The clinic accepts cash, credit/debit cards, and personal checks. If financing is needed, the clinic offers third-party financing options.
Repayment
Refunds will only be given for services not rendered. If a refund is requested, it will be processed within 30 days of the request.
Urgent Care
The clinic offers urgent care to current patients during regular office hours. Patients should call the clinic for instructions on urgent care after hours.
PRIVACY
The clinic respects the confidentiality of its patients and will not disclose any personal or medical information without the written consent of the patient.
Limitation of Liability
The Clinic shall not be liable for any damages resulting from the use of its services or website.
Changes to Terms and Conditions
The Clinic reserves the right to make changes to these terms and conditions at any time without notice.
Applicable Law
These terms and conditions shall be governed by and construed in accordance with the laws of the state/country in which the clinic is located.
Dispute Resolution
Any dispute arising out of or relating to these terms and conditions shall be resolved by binding arbitration in accordance with the rules of the American Arbitration Association.
